 PSA funeral benefit

The PSA renders financial assistance with funeral costs in the event of a member's death.

The provision amounts to R2 350 per member and is granteed on an ex gratia basis, that is, as long as the funds are available. In the event of a member's death, the dependents/next of kin must inform the relevant PSA workplace, branch/committee representative or the PSA Provincial Office and submit the necessary duly completed forms to the PSA Provincial Office.

On receipt of the completed, prescribed application form, the PSA Head Office will effect payment, provided that the application is received by the PSA within six months of the date of death.

DreamBuilder Family Funeral Plan – offered by Group Schemes, negotiated especially for PSA members - providing you with the following benefits:
Funeral cover for your whole family.
 
Choose funeral benefi­ts for yourself, immediate family (life partner and children), parents (maximum of four parents) and relatives (maximum of nine).
The amount you pay for your cover will depend on the number of people covered on your policy and their ages (parents and relatives must be younger than 85 years).
No more premiums are paid after your death, disability or retirement.
All the family members you cover on your policy will still be covered after your death, disability or retirement. They will not have to pay any further premiums.
Add cover for your extended family members or choose extended family cover on its own.
 
The Family Assistance benefits are optional and available to everybody covered under this policy.
All participants covered under the scheme have access, 24 hours a day, seven days a week, to the following services:
  • Repatriation of mortal remains: assistance with the moving of the deceased’s remains to a funeral home closest to their normal home.
  • Funeral Assistance benefit: assistance with funeral and cremation arrangements.
  • Legal assistance: a 24-hour helpline to assist with legal matters manned by qualified attorneys.
  • Emergency medical services: 24-hour emergency medical assistance for injured participants.
  • EA Touch: a 24-hour emergency assistance helpline for information and arrangement of treatment for trauma, assault and HIV infection.
The Income benefit provides an extra amount of money at death.
This optional benefi­t  will be paid every month for a period of six months. The cover you selected will be divided into six installments.
Keep your funeral cover up to date with rising costs (inflation).
This optional benefi­t allows you to select a percentage which will increase your premium and your cover level every year.
Continue with your cover when you cancel your membership of your organisation or union or when you resign.

IMPORTANT INFORMATION
·         All premiums shall be paid monthly in advance
·         Due to this product being a GROUP FUNERAL COVER product, cover works on a month-to-month basis.
·         Premiums are payable by Debit order or Persal stop order.
·         When Persal stop order is the chosen method of payment - The official Persal authorization form must be completed and signed by the premium payer and submitted with the application form.
·         Notification of claims to be within 6 months of a member’s death.
·         Failure to do so within 6 (six) months will result in the benefit being forfeited.
      ·     In the event of death a Claim Notification Form must be requested from Zisekele (0861 88 00 00) and must be submitted together with the  relevant supporting documentation.
·         The turnaround time for a claim is 48 hours on receipt of all required and completed documentation.
·     Zisekele Group Funeral cover is underwritten by Sanlam Developing Markrts Limited and administered by Phakama.

Vehicle access benefit by Avis.
For an additional R15.97 per month, in the event of the death
an immediate family member, a vehicle will be available for a
