
PSA funeral benefit
The PSA renders financial assistance with funeral costs in the event of a member's death.
The provision amounts to R2 350 per member and is granteed on an ex gratia basis, that is, as long as the funds are available. In the event of a member's death, the dependents/next of kin must inform the relevant PSA workplace, branch/committee representative or the PSA Provincial Office and submit the necessary duly completed forms to the PSA Provincial Office.
On receipt of the completed, prescribed application form, the PSA Head Office will effect payment, provided that the application is received by the PSA within six months of the date of death.

Excellence Family Funeral Plan

This plan can be used to provide your whole family with funeral cover. Dependent lives that may be covered under your Excellence Family Funeral Plan include your spouse, children (adopted and stepchildren included), parents and parents-in-law and any other family member such as uncles, aunts, brothers, sisters, nephews and nieces, that are financially dependent on you.
BENEFITS
· The Funeral Benefit is the main benefit available to all Lives Assured and provides a lump sum on the death of the Life Assured.
· The Accidental Death Benefit provides a lump sum on the accidental death of the Funeral Benefit Life Assured (all Lives Assured qualify for this optional benefit).
· The optional Family Income Benefit provides for the payment of 10% of the Principal Life Assured's Funeral Benefit sum assured on a monthly basis for a period of 6 months on the death of the Principal Life Assured.
· The Premium Waiver Death Benefit is automatically included. Should the Principal Life Assured die, the Funeral Benefit and elected Accidental Death Benefit will be provided for free to all the Lives Assured on the Policy for a period of 5 years.
· The Cashback Benefit may be added as part of the Excellence Family Funeral Plan.
· Assupol On-Call is automatically included as part of the Excellence Family Funeral Plan. The Assupol On-Call benefit includes: transportation of the deceased, guaranteed hospital admission and medical, financial and tutor advice lines.

Absolute Advantage Family Funeral Plan
Assupol Life expanded its well-known family funeral plan to include more than just funeral benefits. At Assupol Life we believe that our policyholder should benefit from their policies while still alive.
A choice of funeral cover for yourself, your spouse and your children between R3 000 and R20 000 is available.
The following optional benefits may be elected:
· No Claim Bonus benefit after 3 claim-free years.
· Accident Death benefit is available for all Lives Assured.
· Premium Waiver – premiums cease should the Main Life Assured pass away.
· Family Income Benefit – R1000 per month is payable for 6 months after the death of the Main Life Assured.
· Your policy may be linked to inflation. Premiums will increase by 10% per annum and benefits by 6% per annum.
· Assupol On-Call offers services such as Trauma Counselling, Medical Advice, HIV/Aids Information Advice Line and Transportation of the Deceased.
· Extended Family Members up to the age of 80 years may be added to the funeral plan.
The Paid-Up feature, whereby premiums cease at age 65 of the Policyholder (and all benefits remain in-force), is automatically included at no additional cost if the Policyholder is younger than age 55 at the inception of the Policy.

- Members enjoy cover as long as premiums are paid
- Premiums determined by chosen plan and age next birthday of main assured at inception of plan

- No death benefit is payable when death of the assured is as a result of suicide or HIV/Aids within two years of inception date
- No waiting period when death is as a result of an accident

IMPORTANT INFORMATION
· All premiums shall be paid monthly in advance
· Due to this product being a GROUP FUNERAL COVER product, cover works on a month-to-month basis.
· Where premium payments are missed, a double premium will be requested the following month. If the double premium is also missed the policy will lapse. Should the client wish to reinstate this policy in other words resume with the premium payments, it will be treated as a new entrant with the applicable waiting period
· Premiums are payable by Debit order or Persal stop order.
· When Persal stop order is the chosen method of payment - The official Persal authorization form must be completed and signed by the premium payer and submitted with the application form.
· Notification of claims to be within 6 months of a member’s death.
· Failure to do so within 6 (six) months will result in the benefit being forfeited.
- In the event of death a Claim Notification Form must be requested from Zisekele (0861 88 00 00) and must be submitted together with the relevant supporting documentation.
· The turnaround time for a claim is 48 hours on receipt of all required and completed documentation.
· Zisekele Group Funeral cover is underwritten by Resolution Life Limited and administered by Phakama.
